Try flushing it first. Use a garden hose to clear the heater core. Take the supply and return lines off keeping the ends above your engine so they don't leak all of your coolant everywhere. Use the garden hose on the return side to push any possible loose blockages back out through the supply line. If somebody has ever put Bar's StopLeak in your radiator, that would likely be the culprit. I have done this with a few trucks with success. If it's a total blockage, I'm afraid you will be spending a weekend removing components to replace your heater core. Good luck.

EDIT** All of this flushing is from the engine bay with the engine off.

trace those hoses back to the engine. The hose that is closest to your thermostat would be your feed line into the heater core. The other hose than would be your return line. That is the line you'll want to flush the heater core with. Un plug both hoses from the engine, and backflsuh it through the return line. The idea is to put pressure through the opposite direction to flush anything out. Also, you can use compressed air to back flush as well. I don't know how much PSI you should use if you go this route, but I wouldn't use a lot as you might damage something.

Could also be your blend door. A lot of the time the plastic teeth that lock in to the motor that controls the blend door get brittle and crumble apart rendering the blend door useless. They make replacements that have metal rings around them to prevent this from happening, or it has the teeth replaced as a metal insert to make install easier. If you were to do the one with the metal insert, you could just take your glovebox out, cut a hole in the plastic behind it (it's just there for looks, nothing structural), then cut a hole in the HVAC box on the left side to gain access to the blend door. you then remove the old broken blend door and put the new one in, but you have to put the metal insert in from the bottom, meaning you have to remove everything from under the dash in the middle section over the trans hump, then unbolt the motor. You can search for a video of a guy who did exactly this on YouTube. Video is about 15 mins long iirc.

Take a picture of the firewall where the hoses go to. There should be clamps on the hoses themselves. They may be swedge clamps instead of the old screw type hose clamps.

And I forgot about the blend door. Can you change the direction of your airflow? Like go from defrost to vent to floor? If not, you've got problems other than your heater core.

IIRC the defrost setting is the default setting. I know my wife's old explorer was a vacuum operated actuator. Could be a loss of vacuum that allows it to switch. Might check your vacuum lines and wiring under the dash.
