Default toasted engine?

So my truck started acting really crazy and was shaking pulled codes and being told that's the piston is shot. Is that true

Ouch... Looks kinda like yes. What engine is that in? If it's an 04-08 5.4 then that's an incorrect plug. Other than that you could try feeling the head with a pinky inserted through the plug hole to see if there's any indication of the piston hitting the head.

Plugs can blow out of the heads on these engines, which is what it looks like. I'd fix the threads, put another pug in and see what happens. I doubt the piston is shot.

Your plug worked loose and spit out at last couple of threads. Not uncommon at all. Put a heli coil in and call it a day, I'm sure the piston is fine.
