Both of those codes can also be related to a dirty MAF. You can remove the MAF sensor and clean it with MAF cleaner.
To check for vaccum leaks, you can use butane or carb cleaner. If you hit a bad vac line, the motor will suck in the butane and cause the idle to increase. Then you know you have found the leak.

Do what Stang suggested first, and then see if the misfire on cylinder 5 disappears. It may be that you're getting a misfire because of your lean codes. After that, you can start moving around your cylinder 5 spark plug and cop to another cylinder to see if the miss moves.
