Gives you the option to inspect diff anyways and swap fluid too

How long does a bearing usually last? My truck only has 142k on it

Who knows rear it's all based on maintenance and luck basically. What you use the truck for etc

But seems like the noise is only coming from the drivers side of the axle. Almost like its inside the drum.

It is, geeze right off he said he had an open dif. My fault for not reading the first post. Your correct as well, they do that noticably when turning. That vibrating vs smooth slipping like they should.

Are you sure it's open? Your not an H9?

Surprised that a 9.75 got stuck with an open rear end.

Anyway, IME, I've seen the shaft bearings leak, like eveyone else, - just haven't heard them making noises before. But, then again I've heard bearings make all kinds of peculiar noises in that past lol, sealed bearings anyway.

Is the pinion seal wet around it ?

Well, you just posted you have drum brakes on that 99 (sorry long post, I'm eating a chicken and watching a movie as well.), - and that the noise may be coming from inside. Honestly I didn't know any of the 99's had drums lol.
Never the less, yea, inspect the brakes first of course, that's a definite possibility given the symptoms.

If that's not it swap out the shaft bearing...just one side at a time, that's important. Inspect the housing and tubes for tooth chunks or parts and check all the teeth.

Make sure you have good fluid in there and enough of it, - you know what they say about hygiene

The pinion seal is not wet, and the brakes and rotors look fine. Just seems like the noise its making is very close to the rotor.
