Hey, I'm not that old!!!
To the OP. did you run the engine (briefly) without the belt on to see if the noise went away or does the truck need to be moving also? I watched your vid. That is a gawd awful noise for sure. Just trying to narrow it down...Could be driveline related or possibly even the alternator?...

I want to test it with the belt off to see if it is within the belt line.
I have to date replaced an idler pulley, tensioner pulley, tested the alternator (passed with no noise x3), removed oil pan (nothing residual in the oil pan), put it all back together and still making the noise. Next step is testing with the belt off but i have to hit the gas to get it to make the noise, will this hurt the engine if i do this for a min or two?

I've heard and seen an air box making sounds from not being sealed correctly, but this sounds worse.
And you can usually run the engine for a coupe minutes without the belt with no ill effects. Just watch your temp to make sure you don't overheat it.

After listening to it again, it does sound intake related. If it were a bearing or pulley, it would do it after letting off the throttle. Check your intake system for cracks or bad seal.

Did you check the foam seal on your air box like I suggested? Doesn't sound like that's your problem to me but it only takes like 3 seconds to check it.

I'm no expert, but to me that sounds like a belt. Like one of the pulleys is not spinning freely enough and the belt is slipping. Does the belt look worn at all? You said you noticed a slight squeal in the alternator when you spun it by hand. Are you sure the Autozone tester is putting the same lateral load on the pulley that it experiences in the truck? And have you tested the truck with the AC on and off? Shotgunning here...not sure if its helpful or not.

Passenger Manifold Leak!
ok so for an update. I ran the truck without a belt and the screeching was still there. I took it to a shop to get a diagnosis, the truck has an exhaust leak from the passenger manifold and its pretty bad. The quoted me 950 to replace the manifold, i told them no thank you and took it home. I have all of the parts needed to complete the repair here at home.
Once i dug into it and started to soak everything i found that the front two bolts were broken off already. so 2 of the 8 are already broken, not to excited.
I attached a picture of the broken bolts that I found. I am trying to remove the AC compressor so i can get a little more clearance. Also i will be lifting the engine an inch or so as people have recommended.
Does anyone have any ideas for drilling out these two front bolts that i cant even touch with my hand?

So, you have replaced the belt and idlers and triple checked the alternator, etc.... Those would have been my first guess... especially after listening to your YouTube video. The squealing happens when you sharply increase the engine speed (gun the motor). This is the classic symptom (and sound) of belt slip... especially with old style V-belts or possibly bearing (or bushing) torture. Try some belt dressing on the new belt. It's cheap and quick. Like a buddy of mine says: "new does not necessarily mean good". You might have a bad belt or some sort of offset in the path since either of the idler bearings could be making the noise. Maybe one of the idler bearings is dry. When you run out of places to squirt WD-40 or stick on duct tape THEN you break out the tool box.
Oops, I just re-read your post and it makes the noise with the belt off. Hmmmmm. The only thing thats turning then is the crankshaft pulley, right? (and all the internal engine bits) I wish I could see your engine in the video. I have a 2V 4.6 and they are much more simple. Sorry, I'm not much help here.
I have nothing for you on the header bolts. What a PITA. Is it time for the 'hot wrench'?

I have the exhaust fully disassembled. All 4 y pipe flange bolts came off pretty good with some squeaking. Tomorrow i will start pulling the manifold.
I pulled the whole exhaust because i am putting a new muffler on and new cats. it should sound awesome when it is back together.
I only hope that this solves the ridiculous noise coming from the engine compartment. But im not keeping my hopes up.

Just to update everyone, I finally figured this out and fixed it. The screeching was coming from the passenger manifold. The cat was clogged and the pressure was screaming out of the front of the manifold. The front two bolts were already broken. Nightmare to get it out. I jacked up the engine and got a drill on it and then broke an easy out. After drilling for what seemed like forever I ended up pulling the engine. PITA! but finished. Replaced all plugs and intake manifold gaskets. The front two bolts on the driver side were busted as well. since the engine was out already though that was pretty easy. both manifolds were replaced. Cats were replaced. she can breath now and is running amazing. Thank you for all of your help.
