My wife started having Bluetooth volume related issues in her car when her phone had an IOS update. She Googled it and found information about setting the Bluetooth setting on the phone as a speaker. She just did it a few days ago and hasn't said whether it fixed the issue. I use Android so can't comment further.
Not sure if this is related, but might be. I had the same issue with the volume of the various nav announcements like upcoming turns (Ford NAV, Google Maps and Apple Maps) with CarPlay in use.
It seems as if there are 3 different volume settings:
1) Audio Volume - stereo/music stuff. Changing the volume when listening to the radio, XM, etc, pop up window says "Audio Volume".
2) Phone Volume - Volume of phone ringer and the voice audio during the call. Have someone call your phone when connected to CarPlay and change the volume while it is ringing. It should open the window showing "Phone Volume". It is the same volume as the audio used for the voice portion of the call. I find that I turn down the ringer but then have to turn the volume back up to hear the caller well. This setting is stored so after hanging up, on the next call it is going back to the volume setting where i had to turn it up to hear the previous caller. Yes, this can startle the heck out of everybody in the cab. My ringer is Party Rock Anthem...!!
3) Prompt Volume - Volume for prompts from things like Google Maps, Apple Maps, Forc Nav, etc... If you want the prompts to be a different volume, you must adjust it WHEN THE PROMPT IS BEING MADE. You should see "Prompt Volume" window. I will go into Google maps and set a destination and select Start. When it is giving the initial instructions, change the volume to the level you want. Once the prompt stops, you will be back to adjusting the "Audio Volume" so you must catch it at the right time.
I hope this might help some as I used to have the same issue. I still do when I forget to turn down the Phone Volume at the end of a call and my music ringtone blast through....
You mean with all the technology in this truck, they can't figure out how to adjust the volume of the ringer without sacrificing being able to hear the caller?? This makes no sense at all.

that would be nice but Ford does not build the vehicles to comply with phones, whose technology changes with every software-update and vice versa.
My iPhone doesn’t ring at all when it’s connected to my truck. The actual ring comes through the truck speakers with a ring tone that I picked in the truck settings. Not sure if this is the same thing you guys are having issues with or not. Sometimes it can be a little loud, but I can instantly turn the volume **** or hit the steering wheel controls to turn it down just like addabob described above. I usually turn the volume up when talking to people then try to turn it down before I hang up so the next time it rings it’s not so loud.
When my phone isn’t connected to my truck it rings like normal and it’s usually on a very low volume.

Yes. It makes no sense and is the most annoying part of the truck. I never remember to turn the volume down a bit at the end of a call, so the next time it rings it blows my ear drums out.
Having the same problem ever since I went from iPhone 12 Pro Max to 15 pro max. Ringer volume resets to max volume after wireless connection to CarPlay. Very annoying
Following. Interesting this thread is nearly 2 years in the running. Just traded in our 2021 F350 Lariat to downsize to the 2021 F150 XLT Sport somethingerother (got rid of the fifth wheel, didn't need the F350). The text alert in the F150 is a legit safety issue - it DINGS at volume 100 and nearly makes me swerve into the ditch when that danged thing fires off. The first time, I was like, "What..WHAT?!? WHAT WAS THAT?!?!?" I thought maybe it was the truck telling me I hit a dog or "watch out for the little old lady" or something. Turns out it was a text message alert. Good grief! The 2021 F350 just interrupted my audio for a second, then a polite "ding".

My ring tone is Party Rock Anthem, and it never fails that whenever my wife and I go somewhere, I get a call. Startles the living crap outta her, and me too if we must be honest…. I turn it down, but then the volume is too low to hear the person on the call so I turn it back up…. I sure wish they could figure it out. Not sure if it is a Ford Sync issue, Apple CarPlay issue, or something specific to Ford’s implementation of CarPlay.
might have to look at the FORScan docs to see if there is any way to set the volumes to some default level below eardrum-damage level….
