It could be several different thing,it would be best to pull the top cover off and look
To see what really wrong with the tranny. It could be a bend shift fork, reverse gear,
Or the reverse shift fork its self. I'd pull the top cover and look to see whats wrong.
Southwestgear. Com has all the parts daigrams for these trannys on line. Good luck.

Like whats been said could be any of those issues, this is a problem you are going to have to take off the top cover to find out. If it is in fact a bent/worn shift fork for the 5th and rev, the transmission is gonna have to come apart to get to it.

You can pull back the flooring in the cab to get to it without dropping the transmission. Be sure its in neutral when removing the top cover.

Take a look yourself first. It's easy and you won't hurt anything.

For one you are looking for any bent or worn metal components. On the top cover is a little metal slot at the end of the shift rail that works the 5th/Rev shift fork. If its work or broken that may be an issue, if the fork itself has allot of wear from that slot it won't shift properly. The fork can also be worn where it rides on the collar.

Here is a tear down of what you should be looking at. Notice item 16 in first picture. If it does end up being the 5th/rev shift fork, the transmission will need disassembly.
