Default Rear Disk Conversion

SO i'm looking at having to do a complete rebuild of the rear brakes on my 97 F150 XLT. Everything is worn down and needs to be replaced. Has anyone done a swap to disk brakes. I searched but couldn't find anything. I've done this on Jeeps before but not on a F150. Hell I dont even know what model axle I have. I guess I am starting from scratch and any info would help.

'99 was the first year for rear disc brakes and it was optional on all models except Lariats. Even my dad's "value edition" XLT had rear discs. I've seen 00'+ with drum brakes but mostly on XL's
