A lean code (P0174 for example) means that the a/f ratio is less than desired. This is usually caused by the MAF reading being off or air is getting around the MAF unmetered. So the O2 sensors are saying "whoa I'm telling the PCM how much fuel to put in and we're still lean, what gives MAF?" and the MAF says, "I don't know, I'm sending you what signal I think is right but if I'm dirty or air is getting around me I can't tell you the right things to adjust."

And this has been another episode of If Cars Could Talk. But that's what goes on usually in a lean code.
