That wouldnt cause the truck to go lean, just set off the knock sensors if anything. Hes probably not running enough timing to even trip the knock sensors if that were the case.

Perhaps, but it is still a valid question. Knock sensors are not infallible either.

If the truck is fine under stock tune, I doubt it has anything to do with the truck as it is.
