its the gears in the axle(s). the number is the number of turns the drive shaft must make in order to make the wheels turn 1 time. fully written out it looks like this 4.56:1 or 4.56 rotations of the drive shaft to spin the tires once.

think of it like a mountain bike. when you increase the gear size as your riding, you can have a higher top speed but it gets really hard to peddle. if you go with a lower gear, its really easy to peddle but top speed drops.

basically gear increases torque at the wheels. the higher numerically you go, the more torque you gain but the more top speed you loose. if your truck is lifted and running bigger tires, thats like going with a higher ratio on the bike. you now have a higher top speed because one rotation of the tires make you go farther but because they are bigger, its harder for the motor to spin them. most people regear to gain back the torque they lost from putting bigger tires on. some regear just to have more torque for towing or racing even when they still run the stock tires

and by the way, if you regear, go to 4.56, 4.10 is a waste since more then likely your stock gears are 3.73

parts and labor usually are in the $500-$600 per axle range. if your 4x4, thats 2 axle, 4x2 is just the rear

You can typically regear the rears without regearing the fronts BUT DO NOT use 4x4 if you have two different gear sets front/rear.

Either disconnect or remove the the switch until you swap out the fronts

Typically people can see $600 an axle with gears, install kits and fluids, some see 600 just for install

Wayne at diffsonly will hook you up on a good deal
Go for 4.56 gears with 35s

I'm running 285/70/17 "32-1150"on mine with my stock 3.55 gearing what would be a decent gear ratio to jump to especially with the anemic 4.6l

4.10 gears would probably be suffice but for the cost it may not be beneficial as your only running 32" tires as opposed to the stock 30s id guess you had

I feel like anything more is overkill, especially 4.56 gears but she would move damn quick!
if you could find a set of 4.30s to go on I would go to those
