Been reading where some are seeing a connection between failed emissions testing and the fuel pump driver module being oxidized/in bad shape. Looked at mine and it is all messed up so I ordered one from Amazon. Will let you all know if this solves the issue.

Replace the Fuel Pump Driver Module because I had read it was a potential solution. It worked! I'm now getting only one incomplete reading which is passable in Virginia.

In fact, I drove it 300 miles home from college yesterday. The diagnostic said these four things failed to provide a reading:

Catalyst Monitor - Incomplete
Evaporative System Monitor - Incomplete
Oxygen Sensor Monitor - Incomplete
Oxygen Sensor Heat Monitor - Incomplete

It is also not throwing ANY codes whatsoever, hooked the reader up to it and it doesn't have any current codes or pending codes. Truck is completely stock.

Has anyone else had this issue? My mechanic (ASE Ford) said he has never seen this before with a vehicle that hasn't had recent issues or the battery unhooked recently. Truck has just under 113k on it. It's a 2005 FX4 with the 5.4

Feels like the truck runs a lot smoother. Idk why but it seems like it.
