Could be many things.
Some more info will be helpful.
What year?
What engine?
What symptoms?
Any other codes?
Maintenance records?
COP change?
Spark plug change?

How many miles ago? Did you replace the boots and springs on the coils too?

Oh, wow. Did you get the broken pieces out? From the looks of it, it was like that for quite some time.

what brand plugs did you install ? with 250k miles on her i would look into replaceing all the coils and springs and boots.. and if you did not install motorcraft plugs do that while your at it.. a flashing CEL is a classic sign of a mis- fire condition.
