Default 04 F150 FX4 - safe mode

Hello I've got an 04 f150 with a SAFE MODE issue,
I've replaced both TPS sensor and the throttle body motor which fixed my issue for about a month, now it's back in safe mode and can't get it back out. I replaced both the throttle body motor and TPS SENSOR again but this time it keeps going back into safe mode. Yes each time I reset the computer and go thru the cycle procedure before starting but the throttle body just ( clicks ) does not cycle thru.
I even swapped out the APP sensor ( pedal )
please any ideas & advice is welcome.

The last time I had a failsafe mode issue, it was because I forgot to plug the MAF sensor back in.

Default MAF sensor

I plugged all connectors back in, but yeah I did that to the first time.

Weird. Last time I hit failsafe...was because I hit a giant mud puddle and...*ahem* "hydrated" my CAI.

Default Scorge ?

I'm not sure what you are saying, sorry I'm new with all this, but my problems did get worse after going thru some mud holes , I barely made it home and now all this . Any tips? What did you have to do ?

He's saying air got sucked into his intake. I'd guess it fouled his MAF as well.

Ohhhh...
So, my cold air intake box is exposed. I hit a large puddle, soaked the intake and effectively choked the motor with water. It went into failsafe mode. Had to remove the entire intake assembly, clean and dry it. Had to removed and clean throttle body and immediately changed oil. Voila

Not sure what set-up you have....but...
You could have some nasty business gumming up your intake system and keeping the truck in limp mode.
