I have 35 inch tires with a cat back exhaust and a CAI. When I installed the tune, it woke the truck up. Night and day difference.

x3 on the tune. My truck isn't lifted, but I have 5 star tunes on my 2010 5.4 4x4 with 3:55 gears. With factory size hankook dynapro atms I get anywhere from 18-20 mpg on long road trips with the bed fully loaded (going to the beach). Whenever I pull my bass boat I get around 13 mpg with the tow tune. I do have a dual exhaust in combination with the tuner but no CAI. From all the research I've done the CAI shouldn't offer any gains over factory airbox. I still am possibly interested in installing an S&B eventually to at least get a louder induction sound, plus 5 star tuning will update your tune for free if you purchase from them.

One other thing you might want to research is the weight of your tires. I have been thinking about upgrading my tires, and I am very conscientious of the tire weight. Not exactly sure how much of a difference it will make, but I know it can pose some difference. One other thing I might should add is the tuner like others have said can adjust your tire size to correct your odometer and shift points. That should yield some mpg gains, and at least you will know your true mpgs hand calculated.
