Our Gen. 1 top mount was a static mount, the shock main shaft precisely fit through the top mount with washers top and bottom, and then was bolted down with the 19mm nut. Everything is fine if the shock goes up and down as it is supposed to. However, if you were to hit a large pot hole or bump hard enough the tire/wheel & lower control arm are driven violently backwards (the lower control arms have bushing that allow the LCA's to move)...it can shear the Boss main shaft under the hard mount top mount. This takes a pretty hard hit on something, but it has happened and we had to do something. We acted immediately to fix this issue, and for anyone that it did happen we replaced their Coilovers.
As with any company, we always want to improve, so we came up with Top mount Gen 2. We enlarged the main hole in the top mount then dished the top mount to accommodate large bushings top and bottom with cup washers to hold them in place. The result was fantastic. The shock when mounted in our Gen 2 top mount and the top mount in a vise...the shock can articulate from 10-2 without any troubles. Problem solved.

I know this is a two year old thread but I just investigated a squeaking up front on the passenger side and have the same issue as the OP. Bottom washer has plowed through the shock rod and is now hanging below and truck front end is now uncontrollable when going over bumps in the road here in Pennsylvania.

You need a new cup washer & bushing set. This is not the same issue as what was explained in post #21.
