Does LTE modem require a separate data plan?
It is its own mobile # through AT&T. Obviously not for everybody, but it is an extremely useful feature. It gets better reception than your phone due to the higher powered antenna, I'll commonly get truck wifi but no phone signal in remote areas. If you have an unlimited phone data plan and live in an area where getting a signal isn't an issue it may not be as useful.
I use it to upload large files regularly when working on the road, and my wife can work remotely with it while I drive when we head out for weekend get-aways. It's also nice to not burn up the phone data plan when streaming music, which I do a lot.
