With the new laser cut keys, the dealer requires the VIN and ID to prove ownership. They then order the key directly from Ford. The card is only for the keypad entry. If you have a key, a locksmith with a sidewinder key cutter and the proper blank should be able to cut you the key. Ford wants approx. $85 or more for the key, while the blank is probably around $15 online.

The master code is located under the dashboard just above the brake pedal. You have to lay on your back and look straight up under the dash. It will be on a sticker on the outside of the keylock device. If you don't have the code, the only way to get it is by looking at the box. In other words, if your keys are locked in it and you don't have the code, call a Locksmith.
