Default what would cause this ?????????

I have a 87 F-150 with a 300 i6 i put a new belt and new alternator on it. Well raised the hood today and found this. The belt had got into the fan clutch i check all the pulleys found little play in the belt tencher and about 1/2 of play side to side and front to back on the power steering pump pulley? My question is would that much play in the power steering pump pulley cause the belt to come apart and wrap around the fan clutch? There are some picks. Any help would be Greatly appreciated!

Just a thought but maybe the pulley on the alternator might be wrong. Put another belt on there and see if anything looks out of kilter. Turn it over by hand and see if the pulleys are staying straight.

Looks like age caused his. And a ****load of luck that it didn't fail completely. Actually... is that picture a joke? Or is that really how your belt ran?

I ran it like that, it threw again and I put it back on. Now I have a new belt. Changing the power steering pump kept it from throwing as much but still did some. I think a new tensioner will fix it the rest but I haven't done that yet to make sure.
