1991 4x4 E4OD from hell
1991 F150 4x4 5.0 E4OD 60k original miles
Had it for about three months and lost reverse and neutral. Verified that the tranny was bad, pulled it and had it overhauled. Replaced the valve body connector and repaired all the other connectors while it was out. Did a complete flush and replaced the TC.
Reinstalled it and it worked fine for about a month, then lost reverse one day out of the blue. The next day reverse worked although I did nothing to correct it.
Truck overheated due to stuck thermostat and broke the bottom bolt off in the intake, so had to R&R the intake to fix it. After reassembly the truck would start and idle fine, but wouldn't pull it's own weight in gear.
Replaced cap, rotor, wires, Ignition Control Module, Intake Temp sensor and found a couple of vacuum leaks.
KOEO Codes at this point were: 637, 621, 622, 629, 626 and 624
Re-seated/cleaned every connector on the drivetrain including the speed sensor.
KOEO codes now are: 622, 629 and 624. KOER codes are 538.
The truck runs great and seems to use all gears, but shifts so hard that it chirps the 33s going from 2nd to 3rd.
Removed the ECM and replaced the three electrolytic caps, two of which were obviously bad. Reinstalled and took her for a spin. I had one medium hard shift from 2nd to third, otherwise the 15 mile trip with several starts and stops went great, I thought it was fixed. When I got home I tried to back into the driveway and it wouldn't go into reverse AGAIN!!
KOEO codes are 622, 629, 624 and KOER codes are 311 and 538.
This has been a daily three week process and I am at my wits end. I feel like I have read everything there is to read on the E4OD and the ECM and the only thing I can come up with is to replace the ECM and drop the tranny pan and check and see if the filter has fallen off.

Update: After signing up for this forum and typing this post I went back outside and cranked the truck......reverse works fine again. It has to be temp related.
