Is a programmer a safe thing to use
This is a loaded question. As with anything that you do, if dealt with recklessly bad things will happen.
Depending on the quality of what product you are using, mods, etc all of that can affect your vehicle as well. As well as if you are using some person to write tunes for you that does not know what they are doing there are risks there as well.
There are several tuners and people to write tunes (for the tuners that can be custom) that vary in overall quality and you will find a huge array of personal suggestions.
None of this is meant to scare you by any means, most people claim their tuner to be the best mod they have purchased due to the increased "fun" factor of driving their vehicles even if their is minimal HP gains.
Do your research and find the product that best fits you and your needs.

SinCity is correct. I would say though that simply purchasing a tuner and custom tunes shouldn't hurt your truck or decrease life expectancy. The 2 most widely suggested tuners are Troyer Performance and VMP.
