Question Transmission or not?

2002 F-150 5.4L XLT

Truck had issues shifting into 4th gear (Required extra RPM or would stutter)

After months of this, truck will no longer accelerate. RPM goes up, but only enough power to crawl (3mph) in 1st and 2nd gear. Also drives perfect in reverse.

OBDII Code: Output shaft speed (OSS) senor - circuit malfunction (Control unit: 10). This was displayed when I went to 3500 RPM without moving more than a few feet.

Transmission bad and need to rebuild, am I on track? What would you do?

Yes, You need a trans rebuild.

I would take it to Fred's Transmissions in Hopewell, Va (but I have no idea where you are).

You're absolutely right.

I did some further checks this week. It looks like the forward clutch is bad and the valve body may be jammed with some debris from the snap ring breaking.

Someone quoted the entire job for $2800. Not sure if that's a good price, but it would also include the JMOD and other things.

That's about the right price around here for rebuild with upgrades.
