Default rear passenger side window problem

My rear passenger side window on my 2003 supercrew does not like to stay in the track when it goes up. It has done it ever since we have had the truck and now its gotten to the point where it has cut the rubber seal that is suppose to guide the window up. Has anybody else experienced this problem? and what can you do to fix it?

there are two 10mm nuts at the bottom of the glass that attaches it to the regulator... make sure they are not loose
