Key out, accessories still on
2000 F150 standard with 250k miles, with the key out, the radio stays on, chime ringing like key is in ignition....what to do or inspect first to fix? It's like the truck thinks it's in ACC mode.

Sounds like it's time for a new ignition switch.
Not the same thing, I know, but I had a car a long while ago with a switch so bad I could actually pull the key out while it was in the run position. It was funny to be able to pull the keys out and hand them to the passenger while cruising down the road. This was also the car, coincidentally, whose key would also start our USAF shop step van.

This happened to my 2000 F150 along with some odd other electrical gremlins like the 4x4 light turning on randomly the same day. This really freaked me out to say the least. I disconnected the battery and found some corrosion on the terminals. I cleaned them up really well and the problem has not returned. I have seen in several other posts that these trucks are very finicky when it comes to corrosion on the battery terminals. I know it sounds silly but I think the computer gets stupid if it doesn't pull the exact amount of voltage it wants. Anyway it's a free fix if it works so it can't hurt to try.
