Default 1990 F150 4.9L - Need Help Please

This truck works fine until it warms up, then after about 5 minutes the CEL comes on and it starts to run rough and wants to die. It will run with a little gas pedal but has no power. After it cools down everything is fine again for a little while. I just replaced the fuel filter which was pretty clogged, but that did not fix this problem. My KOEO engine codes are 67, space, 87, and 95. If I'm looking at the right charts the code 67 is neutral safety circuit (5 speed manual xmission), codes 87 & 95 are both related to the fuel pump; primary & secondary respectively. This might make sense given the old restricted fuel filter but I don't understand why it pumps fuel just fine until warmed up? I don't have any manuals or circuit diagrams but am thinking there is probably a relay somewhere that controls the fuel pump but don't know where this is located? Anyone have some ideas?
