Default Undetermined Engine Rattle

So I've developed a rattle recently that isn't there all the time. It happens sometimes after I've coasted for a few second and get back on the throttle enough to maintain speed and/or increase speed a few MPH. It also happens, sometimes, when I take off from a stop sign, stop light, etc. at half to full throttle. In the video below I was able to recreate the sound (minimally) by having it in gear, holding the brake, and punching the throttle really quickly and letting off. It is the rattle noise at the beginning of each rev up. Does this sound familiar to anyone? I can't seem to find anything on here or other sources that sounds like (or described like) what I hear on mine. It almost sounds like the engine is being starved... I've also noticed it sometimes when I turn the key and the truck doesn't start but then I turn the key again and it rattles for a second then starts. Thanks for any help you can provide!

The sound was recorded in the front, passenger wheel well. On a quick observation at the time I couldn't determine if it was coming from the exhaust manifold or within the engine. There was no obvious signs of broken or loose manifold nuts/bolts or leak points.

Check your carburetor butterfly and make sure it isn't sticking at first.

I don't have a turbo, so that's not it.

When I looked over it, I didn't see any signs of blowby on the exhaust and have not heard anything that sounds like a leak.

The "rattle" is more of a mechanical sound (not heat shields either, those are tight). It sounds like it is coming from about the third port from the front on the passenger side.

I have also noticed it is not very prevalent when the engine is cold. When it has warmed up and at full operational temperature I can recreate it almost every time I try. I've tried higher octane fuel, cleaners (haven't physically cleaned the throttle body yet), etc. However, sometimes at "warm" startup after sitting for ~15 minutes it will not start on the first "bump" and then when I hit it again it will gives me the rattle and then finally start.

Maybe a bad injector not providing enough fuel and starving it?

I haven't checked "lately"...I changed the plugs and COPs about 6000 miles ago.
