Question Timing Chain - Tensioners

I have a bad rattling noise coming from my engine, when running at high RPMs it seems to be fine, as soon as you idle or any low rpm driving the noise is pronounced, loss of power to accelerate and the truck visibly shakes.

I originally thought it was the Cat Converter but when I opened the hood its coming more from the engine, around the timing chain, so I am thinking it IS the timing chain, except I know chains are meant to last almost the life of the truck. So the tensioners perhaps??

Anyone had experience in changing the tensioners or have paid for them to be changed. If yes, what's the approx cost I can expect? From what I've read it's no easy job.
