So i bought my truck with 129k, and replaced plugs at 131 or 132, (I have the receipt, just not on me at the moment), it cost me $964, but since my truck was over 120,000 when done i'm not eligable for reimbursment?

So I haven't had mine done yet but have been looking at getting it done at 100k miles or by Feb 2017 which ever comes first. Based on this, the worst case scenarios is things went FUBAR and they have to pull the head costing me $3k. If I read this right, it works as follows:
Total Cost // Reimbursement // Out of pocket
$3000 $1790 $1210
$2000 $1040 $960
$1500 $665 $835
$1000 $290 $710
$750 $165 $585
$500 $40 $460
Still pretty darn pricey to change some spark plugs, but better than nothing I suppose if things go south. Roll the dice and have $1k ready just in case.

I got my notice today...threw it in the trash.

I'm lucky I guess. I bought my 04 in 08 with 55k. Didn't find out about the forum until 2010. Thank god I did though. I found out about the plugs real quick and got after them around 100k.
It's really a testament to this forum and its membership. The guys were really supportive and helpful. Thanks to THIS I easily got 7 of 8 out. #5 snapped, but again, these guys and this forum saved my axx.
I feel sorry for all of the owners that got bent over by mechanics and the stealerships.
It took me 2-3 hours of an afternoon and around $200.
Thanks again everyone!

Just had mine done, at the dealer. Then I got this letter a week later. So it looks like I could get 50% back. Paid $520.00 with 3 broken plugs. I thought the letter came because I got the work done. Weird coincidence.

No, you need to have spent more than $300 on the change to get reimbursement.

I have a hard time understanding the mileage limits. So if you do it after 120k are you eligible?

Yes once again you'll need the long form notice

Thoughts on whether my dealer would provide me with a copy of the original receipt for this service work since I don't have it anymore?

I got rid of my 2005 in 2011 and don't have a receipt, but I had the plugs changed well before 100K miles and at a cost of roughly $1100. The work was done by the dealer that I purchased the truck from, but its not my "current" dealer because I moved a short distance away.

I called the dealership that did my work and they had no problem providing it to me. They emailed it to me as a pdf. I got lucky again because the truck had 119800 on the odometer. 2 hundred more miles and I would not have been eligible.
