What's the axle code on your truck's data plate? I have a feeling most of the 4.6 trucks were geared way too stuff for the amount of weight they have to move.

Also, what fuel are you running? Ethanol? California junk?

My '04 4.6L 4X4 gets 15 to 19 MPG during summer driving all hand calculated and documented in Fuel Monitor App. Almost made 20 MPG one time with no city driving. 90,000 miles, plugs and fuel filter a year ago. Clean the TB and MAF now and then. Stock paper air filter. Tried a K&N for a while - no difference. Fuel is 87 octane Costco.
Tires are larger than stocks - 275 X 65 X 18's.
Axle ratio = 3:55 to 1.
Edge Evo tuner running stage 2.
With a 2WD and stock tires I would be expecting maybe 16 to 21 MPG.

My all factory stock 06 4.6 4x2 avgs 17-19 mpg. It has 165,000 miles and I have never cleaned the throttle body or maf sensor since I bought it new. The first spark plug change was at 150,000 miles and mpg didn't change. Spark plugs looked pretty good other than the gap being a little wide.
