Do all the mirror heads have cut outs for puddle lights? My 06 king Ranch has puddle lights on the pass. side but not the drivers side.

No. Some OEM do not, some aftermarket do not.

They don't all have puddle lights but if you have one from the factory then you should have both. Either someone replaced the passenger mirror and it added a puddle light or someone replaced the drivers side mirror and put in one without a light.
