Offset wheels plus wheel spacers?
The biggest concern with spacers is the extra stress it puts on your hubs and lug studs. Many of the failures you see out there happen after the spacers are installed and the lugs holding the spacer to the hub loosen and eventually the studs shear off. That can be catastrophic. I've known a handful of guys running spacers and they've never had a problem. Some run the expensive Spidertrax, some run the cheapest eBay spacers they can find, none of them have had any problems. But, they all periodically check the torque on the lugs to ensure nothing loosening. Of course, they all run lock tight too.
There is a difference between adapters and spacers. Spacers use the factory studs and just sit between the hub and wheel. Adapters bolt on to the factory studs and have new studs on them. There wont be any issue using a .25" spacer with -12 offset.
