I'm going to take my truck in and see if we can check all of the connections, etc.

EVAP test was a pass
02, Cat, and 02 Htr were failures and the 02 sensors were bouncing back and forth from 0.05 V to around 0.9 V on the graphs.
All temperatures were running at around the right area continuing to lead me to believe that there is a short circuit or faulty connection in one or multiple places, especially considering that literally each of the sensors were replaced just over a year ago.

I downloaded an ios version similar to torquepro with a veepeak obd2 bluetooth reader and it works great. Thank you for the help there!
Your o2 sensors are supposed to bounce back and forth like that so it sounds like that part of them may be wired correctly. I don't know about the heated part is it possible some generic sensor were used and not wired correctly?
i think there may be a fuse or relay for the 02 heater circuit

@Cadehowell I have heard / read very bad, or at least doubious, reports about the IOS (knock off) version of Torque Pro. The developer of Torque PRO (Lan Hawkins) is a "genious" programmer and has developed an unbelievable App ----- but has NEVER (for some reason) transported it to IOS. I am surprised if my CSV file of custom PIDs actually works in it. But all that I am saying is -- beware. It is NOT Torque Pro that we talk about around this forum. I know IOS users are loyal, but we actually advocate around here to just purchase a cheap Android phone or pad - as long as it has working WIFI to download from Google Play Store through your WIFI, and working Bluetooth to connect to a dongle. You don't need LTE or 4G, 5g or anything else.

Are you going in closed loop ? Are the short term and long term numbers coming down . A o2 heater problem should be addressed , blown fuse etc . wiring around manifold gets burnt if you don't protect it ,tie it back . Take some screen shots . But try to talk torque pro ,its cheap and you can get a non activated android for under 20 bucks . o2 are supposed to switch ,yes we look at the volts as well as the other. . You have got to get it in closed loop . .That means its trying to tune itself in not run on default (open loop). It needs to work on how much o2 is not being burnt .
If it has bandit o2's i don't know what you are going to get . Pcm can only adjust fuel to each bank according to its final grade the o2 amount, faulty injectors or ignition can't be fixed by pcm ,only slight variations can be dealt with . the pcm constantly adjusts fuel and cam phasors timing with oil pressure thru vcts . But junk in signals or faulty devices are out of its range. o2 must be heated to work well . Long term failure can burn cats out quickly The pcm is run by the first 02 sensor and adjusts engine the second o2 tells how the cat is doing ,pcm can't adjust cat only whats going into it .o2 age .and go bad after 100k .
your emissions test is trying to determine the same .
