So I did the ugly and replaced the timing a few weeks back I've been busy with work and getting the yard ready for spring so I never had a chance to take some photos. Here they are. Both tensioners had blown seals. One of the plastic tensioner guides was blown into many, many pieces, in the oil pan. The timing cover was ground down on the passenger side as well. If it's slapping, get it done. Mine never ran badly, it just sounded like crap. 2005, 150,000. Top is the right side, bottom is the left. The left side is so worn I can compress it fully with one hand. Mmmmm. Chunks. Drop the pan. You'll be happy you did.
The passenger side manifold was almost broken into two pieces. It took damn near as long to change it.

Mine was similar. Drivers tensioner was fine but passenger side was fully compressed, stayed that way after I removed it. I didn’t even have a chain guide present. Amazed the truck ran fine, the only indication that something was amiss was the 2 second startup rattle and the hot idle knock. It was quiet as a church mouse for the 15 minutes between the rattling ending and the knocking beginning. Having the job ending successfully is a great feeling.
