Default Pulse or surging at slow crawl

Have a rather weird issue and I tried the search function and couldn't really find anything. While stopped and in drive and brake applied....I slowly release the brake to pull forward and the truck will surging/jerk until I get about 3 mph then it goes away completely. If you press the brake and slow the vehicle to a slow crawl the pulse/surging will return. It doesnt do it if you take off from a stop light but only if you do a slow pull forward. It currently has 118k on it and I did a full trans flush at 100k and did it prior to the flush. Any ideas?

And ... Your "vehicle" IS?
