It's more likely fuel pressure-related. Put a gauge on it so you can see it while driving, and try to notice what it does next time the truck runs badly or stalls.

Not likely. The outside of the filter has no impact on how the engine runs. If it runs well, then the inside is probably OK, but it sounds like it's overdue for a change anyway. Before you attempt to change it, read this caption carefully:

That changed to all-electronic in '93, and vacuum hose leaks wouldn't make the engine stall anyway. They'd make it idle high & maybe rough, but they wouldn't affect driving enough to notice.
