Guess I need to read closer, I thought we were talking about the frontend, my bad. Well, I guess that points to something bad/gone in the rear end. At least you've got an idea now.

If there are very fine metal shavings showing up on the fill bolt for the rear diff what parts are likely damaged?

Have you hauled really big oversize too heavy loads or way too much tongue weight ? That will punish the rearend bigtime.

I did once, but to be honest I can't remember if this noise started before or after that. Although the noise didn't happen for months after I bought the truck the previous owner did use it for a bit with a plow on it.

I couldn't say until the cover was removed, metal floating around back there is never good.

I wouldn't be to worried yet. First thing I would check is backlash, second thing I would check is pattern. It sounds like your backlash may be on the verge of being a bit to tight, hence why you experience noise under load but not during other conditions.

Sounds like the rear end needs to be rebuilt/replaced. If so, I would price out some of the better ones on the market.
