Default Motorcraft Spark Plug Issue

Has anyone else had this happen or seen this?
In December I had my mechanic replace all 8 plugs with Motorcraft plugs purchased from Oriely's. Just over a month ago I had a misfire on cylinder #7 and when the plug was pulled the tip was completely gone, we replaced the plug and the truck ran fine, my mechanic said maybe I got lucky.

Well it happened again yesterday on cylinder #6.
Now I'm worried about the other plugs and what might be inside my cylinders or heads from this.

Could be running too lean or possibly a leaking head gasket allowing coolant into the cylinder, hows the coolant level, have you been having to add any?

Never had to add coolant. Other than the 306 and 307 no other codes on the truck either. Truck runs great, even after this!

With 216000 miles you need to change all the plugs, not just one or 2. They do wear out as you can see from your pic.

Please read original post. ALL 8 plugs were changed in December less than 20k miles ago, now 2 plugs have fallen apart, these plugs are now worn but defective in my opinion.

had this same issue on 7 plug missing it’s metal shroud did u figure out anything over them fixing the issue or who to speak to over this problem cause I don’t want metal in my engine an the coil is new an it looks as if hasent been changed in over 5 years spring only coils don’t go out that much an if they do ur know but I have never in my life I counter a dumb *** mf issue as this this why I stay with chevys had be stupid new better
