Default Inspection Report Card

I'm looking for an original PDF file of the current Multi-point Inspection Report Card (see attached). I'd rather find an original as opposed to a scanned version of the report.

I'm planning on purchasing a new F-150 soon. I like to do my own maintence. I'd like to use the report card to document my maintence efforts. I'd print out the report and keep a maintence log of my own. While a professional certified mechanic is well worth the money. I can do a fair bit of the mainence on the report card. It'd be great to capture the info in that format.

I'm having the same problem as trackdaddy. After a lot of searching on Google and other ford forums, all I have found is scanned forms too. The link in this thread does not work. Does anyone have a PDF or other format that is more clear that a jpg or gif?
