Disclaimer! I'm no modern mechanic--I stopped working on anything more complicated than oil n' filter changes, brakes, and tire rotation when new cars started getting smarter than I was. Hell, I ain't used my old inductve timing light and dwell tach in 15 years. That bein' said....
I had a 2000 F-250 5.4L with a similar problem that you describe: stalling on cold start and poor running on restart, almost like it developed a monstrous vacuum leak that would more or less go away as the engine warmed. I took it to the dealer and they replaced the "air valve" (don't quote me on that; it was 6-7 years ago) which is part of the emission control system and is located at the back of the intake manifold.
Hope this might help.
