Like for a body lift? Lifts the chassis off the frame
Or a throttle body spacer? Left over idea from the days of carbuerators, where the fuel was introduced into the air and the spacer gave the fuel more time to mix into the air, as well as swirling the air. Since the throttle bodies don't introduce fuel, throttle body spacers just swirl the air to not much improvement. Otherwise they would come with them stock
