Replace IWE in pairs?
The dealer will be replacing my LH IWE under warranty (parts on order). Wondering if I should pay to have the RH IWE replaced at the same time. Truck has 48K on it and has been babied its whole life.

The IWE wither works or it doesn't. If the right side is working, no need to pay the price!
The IWE doesn't always fail by itself, usually a cause has contributed to the failure and not both sides would be equally affected.
I'd just let the shop do the warranty repair because not all times is it actually a problem but under warranty they tend to just throw away good parts to test it out. You'd be throwing away money to throw away a known good part -let them waste their money instead.
