I was denied this week for not meeting my state guidelines of 3x for the same issue or 15 consecutive days out of service. I have service records showing 4x (the last one being the final repair attempt required by the lemon law) and one visit lasting 17 days.
Last night I took my truck to another dealer to see if I could get one more documented repair for the same issue, and that dealer pulled up my history to find that Ford doesn't have on file the 17 day repair that was also the 3rd repair. So, it's possible from Ford's perspective that you don't meet the guidelines because the dealership never submitted the warranty papers, which is what happened in my case. Good thing I kept the service records, because I have them for arbitration next week.
The dealer that has my truck now documented that it was misfiring badly last night when I took it in. They tried a few different things and said it might need a new engine. Today, they said it was running normal and performed as expected. Because of that, they are reaching out to Ford for assistance because they can't find anything wrong, but they experienced it themselves that something was very wrong last night. I'm looking forward to my arbitration meeting now!!!

Just to give a quick update on my case. RAV contacted me on Dec 21st, I emailed them for a couple weeks and settled on ordering a new F250 lariat to replace me F150 XLT. The dealer placed my order on January 4th, the truck is built and in transit now. My truck is estimated to be delivered next week on the Thursday the 14th. I plan to reach out to my RAV representative to have them build my worksheet with up to date loan information. I am hoping to have a worksheet I agree with by the beginning of the following week and switch the truck by the end of the week between the 20th and 22nd. So far this process has been really easy to me and the truck order only took 6 weeks total from order date to delivery date. I am very happy to be going from the F150 to an F250 built exactly how I want it. Also in my state they are not allowed to charge me mileage, my truck will have roughly 36,000 miles on it when they take it back. I am some accessories on my truck (leveling kit, 35's, big deflector, mud flags, bilstein shocks and air bags on the rear) and am hoping this will help in my negotiation to get a couple thousand off but we will see what I can negotiate. Anyone on the fence about this process I would absolutely build the exact truck you want, if your dealer can't find it then RAV will approve the order without much trouble.

Yes my understanding is they won't generate a worksheet until the truck arrives, which I prefer anyways because I've made two more paymemts so it would be hard to generate that worksheet without accurate loan pay off info.

Mileage reimbursement is solely dependent on your state’s guideline. My RAV agent told me in my state of NY, Ford would charge for mileage over 12000 if I went buy out. However, with replacement Ford ate my mileage (was 18500 at time of exchange.)

A question for those of you who were denied the first time. Did you have to call and request another case number to try again or just call again? The service manager updated my case from his end to show all the times mine has been in the shop. RAV denied my first request because they didn't see all of this.
