OP stated it happens on a *slight* incline. I presumed it does not get worse as the grade increases. If it does, or commanding more throttle results in an rpm increase of 500 or so, I'd look to a worn LOCKUP CLUTCH. A bad TC (broken vanes/scored shafts) must be replaced, friction modifiers can't help. Bad TC's are mostly fluid leaks (rare, ususally just worn or blown seals), hot transmission temps, higher than expected rpms, or no motion.
