wtf blew a spark plug

I'm a proud owner of a 99 f150 5.4 and had the misfortune of losing a spark plug on the freeway ......why did it happen?

I'm guessing it was improperly torqued down or the threads have gone and the compression literally blew it away. Either way it's s cheap fix. At worst you'll need a helicoil. Check all the others too.
