Default Rough shift into OD

When the truck goes to shift into OD, its very rough, and will jolt the truck a bit.. Same thing applies to my grandfather's '95...Any idea of what it may be?

Its pretty obnoxious, and im sure it can not be good for the truck.

Mine was doing that too - I had the tranny flushed and filled with synthetic fluid and that helped smooth it out a lot. I also ended up replacing the RABS sensor on the rear diff, and the TPS sensor on the throttle body - after that, it's about as close to feeling new as it's gonna get.
