I was removing the radius arm brackets from the frame of a 1996 F150 5.0L/E4OD, extended cab, shortbed 2wd and this was used on the passenger side. Pretty sure this side had been replaced in the past because it had no rivets, just bolts. The driver side was normal, one bolt, three rivets.
Anyway, I'm wondering...what is the proper name for these? And where can I find them?
If I remember correctly from replacing the bushings on mine, the passenger side bracket is bolted on, the driver side bracket is riveted. But I don't recall a plate/bolt arrangement like that one though.
I used all new grade 8 bolts when I put the brackets back on though.

He is correct about this, the passenger side is bolted and the driver side had the rivets. I bought my truck new and that is how mine was put together. I also had single bolts instead of the ones you pictured. Maybe your truck was put together in a plant that did it that way. I wouldn't worry about it. I replaced my diver side and just used the bolts that came with the bracket.
