Default Help reading EEC-IV Codes

Hey guys,
I'm a new owner of a 1994 F150 5.0 4x4 with E4OD.
The truck has 193k, but runs phenomenal. It does shift a little hard, and gets maybe 10mpg. I figure that this truck ought to do better than 10mpg, given that its a regular cab short bed, and that I baby it.

This is my first non OBD2 vehicle. I wanted to post up videos of my KOEO and KOER test and have you guys confirm what codes you see.

Anyone? I'm thinking:
628 during the KOEO Test = Excessive converter clutch slippage
538 = (R) System did not receive "goose" test. - I will repeat this test and try to goose at the appropriate time.

You seem to have the codes and related causes right.
Don't know about the 628 code, but poking around other forums suggests the news is not good.
One suggestion - clear the codes and see whether they come back. Would give an indication whether this is a current problem, or something that happened in the past that may no longer be valid...?

Thank you very much for the help!

THe TC slip issue, I'm unsure of. Transmission supposedly has a shift kit and shifts very firm. The truck passes the TC lock/unlock test (flat ground, lightly touch brake pedal, etc), no problem.

I am trying to troubleshoot my poor gas mileage problem.
