With the shutter, Ill bet its an ignition coil problem. Not very hard to change at all...mine would shutter and it almost felt like a transmission problem and I let it go until the coil went bad. To find out what one, start the truck and let it run and go around each coil and unplug them one by one, when then engine doesnt change tone...thats your bad coil.
A small tip, check ALL of the hold down bolts...I had three that the head was broke off. They cold the coil in place so it doesnt back out. I had to weld nuts onto the threads to back them out...with a little patience you can get a stinger in there.
