2nd owner. The only I know for sure that the previous owner did was the exhaust. I bought it in 2006 with 32K on it. Ive put a little less than 50K on it in the last 11 years. Before i was disabled with heart probs I only drove a mile each way to work. I dont drive it very much anymore except to pull the boat and camper or to haul stuff off in not to mention the relatives that always borrow it when they need a truck. But its still my baby and I'll never part with it. I'll try to get a better photo of that rearend tag.... I'd like to know for sure what rearend it is since I may want to go ahead and make the gear change. Stephen I appreciate all the advice, thank you!

I have an 06 with factory tow pkg, came with 3.73 axle ratio with LS, 9 inch axle. You could go to 4.1 gears, but I think you won't like the gas mileage. You may want to try the 3.73 instead as it's a good compromise between the 3.55 and 4.1. I tow a 5000lb travel trailer and have no problems; climbed 11,00 ft mtns several years ago and was happy with it. I'd try the fluid change again; drive it for about 100-200 miles. drain it and refill it again. It may work and save you some $$$.

Yes as I stated earlier. After you try what I said give it some time. A month or so. Give the FM time to work before you give up on it.

Your axle tag says 9.75 because it IS a 9.75. That is dictated by the 5.4 engine. The H9 door code indicates you have a 3.55 ratio and a limited slip differential, but has nothing to do with what axle you have.
