This happened to me as well. I thought I torqued it down but the very same bolt came out causing the caliper to rub against the inside of the wheel and put a nice groove in it. I replace the bolt and torqued it down... no issues since.

So here's some pics of the damage to the rims, the depth of the groove is about the thickness of a quarter.
